The Expense Worksheet is our starting point to understanding where you are now, which will help us get you to where you want to be.

As you complete the worksheet, you may find yourself second-guessing past financial decisions. Don’t! Simply complete the worksheet as a fact-finding opportunity. The more we know, the more complete and accurate your financial plan can be.

  • Investment account information including:
      • Names of securities
      • Share balances
      • Current statements
      • Original cost
      • Date of purchase
  • Bank account information including:
      • Balances for savings accounts
      • Money market accounts
      • Certificates of deposit (with maturity dates)
  • Estimates of all assets and balances including:
      • Home equity & mortgages
      • Business interests
      • Auto loans, including year, make, and model
        • Estimated market value and loan value
      • Credit card balances
  • Trusts or gift arrangements
  • Retirement account information, including contribution amounts for any:
      • Pension information
      • 401(k) statements
      • Profit-sharing balances
      • IRA, Keogh, TSA statements or contracts
  • Previous year’s tax return 
  • One full month of pay stubs
  • Existing life insurance policies and current premium information
  • Existing health/medical coverage, long-term care & disability policies
